Hospice eligibility
Terminal diagnosis where life expectancy is 6 months or less, should the disease follow its normal course.
Patient’s physician usually makes the referral to hospice, but a patient and or the patient’s family can also self-refer.
Some examples of Hospice eligible diagnosis are: Cancer, Neurological Disorders, end Stage Renal Disease, COPD, Cardiac Disease, Dementia, Alzheimer’s, and Parkinson’s disease among others.
Cost
Hospice is generally covered under Medicare, Medi-Cal, and many private health insurances.

What are the levels of Hospice?
Routine Care
This level of care is provided for the patients wherever they call home. The patient home may include a residential facility for the elderly, skilled nursing facility, assisted living, and independent living. Services include routine doctor, nursing, chaplain, volunteer, social work or home health aide care.
Respite Care
This level of care is provided in a skilled nursing facility for up to 5 days. This allows the family and caregivers a rest period.
Inpatient Care
This level of care is provided in a skilled nursing facility or hospital for patients who are experiencing an acute crisis, such as short term, uncontrolled symptoms.
Continuous Care
This level of care is short-term, in the patient’s home, and performed by a nurse to stabilize a patient who is experiencing an acute crisis, such as uncontrolled symptoms.
