WHAT IS HOSPICE CARE?

Hospice care is holistic care for terminally ill patients. We focus on the quality of life for people who are experiencing an advanced, life limiting illness or disease. Hospice care provides compassionate care for people in the last phases of an incurable disease so that they may live as fully and comfortably as possible.

What are the levels of Hospice?

• Routine Care:

This level of care is provided for the patients wherever they call home. The patient home may include a residential facility for the elderly, skilled nursing facility, assisted living, and independent living. Services include routine doctor, nursing, chaplain, volunteer, social work or home health aide care.

• Respite Care:

This level of care is provided in a skilled nursing facility for up to 5 days. This allows the family and caregivers a rest period.

• Inpatient Care:

This level of care is provided in a skilled nursing facility or hospital for patients who are experiencing an acute crisis, such as short term, uncontrolled symptoms.

• Continuous Care:

This level of care is short-term, in the patient’s home, and performed by a nurse to stabilize a patient who is experiencing an acute crisis, such as uncontrolled symptoms.
