    What is Hospice?


    Hospice Provides specialized care and support for terminally ill patients and their families. Focuses on patient comfort, our hospice team provides care that promotes physical, emotional, and spiritual comfort to enhance quality of life.

    Coverage is provided by Medicare, Medicaid and by most private insurance providers

    Patients are eligible for hospice care if there is a terminal diagnosis where life expectancy is 6 months or less, should the disease follow its normal course. Patient's physician usually makes the referral to hospice, but a patient and/or the patient's family can also self-refer. Patient or patient's durable (power of attorney must be aware of the prognosis and decide on comfort care instead of pursuing curative measures. Some examples of hospice eligible diagnosis are Cancer, Neurological Disorders, End Stage Renal Disease, COPD, Cardiac Disease, Dementia / Alzheimer's, and Parkinson's Disease among others




    Where can I receive services?


    Hospice patients are cared for in their personal residences, assisted living facilities, nursing homes and in contracted inpatient hospital settings.




    What are the levels of Hospice?


    • Routine Care:
    This level of care is provided for the patient wherever they call home. The patient home may include a residential facility for the elderly, skilled nursing facility, assisted living, and independent living or at home. Services includes routine doctor, nursing, chaplain, volunteer, social work and home health aid care.

    • Respite Care:
    This level of care is provided in a skilled nursing facility for up to 5 days. This allows the family and caregivers a rest period.

    • Inpatient Care:
    This level of care is provided in a skilled nursing facility or hospital for patients who are experiencing an acute crisis, such as short term, uncontrolled symptoms.

    • Continuous Care:
    This level of care is short-term, in the patient’s home, and performed by a nurse to stabilize a patient who is experiencing an acute crisis, such as uncontrolled symptoms.
